The ball of your foot should be slightly forward on the peg, and between a half and one inch from the frame. Once again, this allows for easy movement forward, back, or to the side. The sole of the boot should be horizontally level. The pegs should be used as 'pivot points', about which you move your body.
Lever position is a matter of personal preference, but most of todays top riders put them approximately in-line with their arms, while in the standard riding position. Too far down will put pressure on your wrists, while too high up will stress your clutch and brake fingers. Ride with your index fingers on the levers. The only time a finger will come off is when a lot of throttle is required, and the wrist twists too far to be able to keep it on the lever. Many expert riders have the finger out and ready when the throttle is rolled back.
